We ask you to provide your insurance information in order to verify your insurance benefits for you before your appointment. If finances and insurance benefits are a concern, we will gladly submit a predetermination of benefits to your insurance after you have had an initial examination and treatent plan. If you need financial help, we are able to help you spread your payments using a line of credit from Care Credit in just a few minutes.
